Please follow the format carefully. As an Adult Gerontology Nurse practitioner, Write Case log 2 using the format below. Attached will be the case to use for the case log 2 and I will also attach an example of a case log done prior semester.
Case Log format: Attached is the Case Study and an example of a finished graded case study paper
Format:
Part 1: Briefly describe the case and why you selected this patient, 2-5 sentences
Part 2: Write up the case according to case presentation format: Chief Complain, History of Present Illness, Histories (family, medical/surgical/psyche employment, social), Review of Systems and Physical Exam (only include those pertinent to the case).
Part 3: Differential and actual diagnoses: Student must discuss evidence, decision making and how the diagnoses was reached.
Part 4: Plan: The plan must be comprehensive, document decision making, and rationale and siting evidence. State why each part of the plan is in place. It is not sufficient to say why medication is prescribed you must address your selection ie. Antibiotics are prescribed to treat infection, but why did you select Penicillin.
Part 5: Theory: Apply a nursing theory to your plan. This can be done in 1- 2 paragraphs
Part 6: Cultural and Ethical influences: Which special population does this patient belong to, how that influenced your plan. Was there evidence about these influences? Does this case raise any issues of ethics?
Part 7: In each log, indicate which course objectives, NONPF and QSEN competencies are pertinent, and how you met the stated competencies during that clinical experience.The specific competencies that you are addressing should be stated at the top of the discussion as a framework for your description.
Discuss anything else that is pertinent to the case. The reader should not be left with questions!
